Blepharoplasty for drooping eyelids

An examination must distinguish loose skin from a low eyelid margin, brow descent or nerve and muscle problems. That distinction determines whether eyelid surgery is appropriate or whether another evaluation is needed.

Dermatochalasis describes redundant eyelid skin that may fold over the crease and sometimes weigh on the lashes. True eyelid droop involves the lid edge sitting lower than expected, while a lowered brow can push skin downward. These features can coexist but are treated differently.

The clinician also checks whether the concern affects peripheral vision, whether both eyes behave similarly and whether dry-eye symptoms are present. Tear trough hollows below the eye are a separate contour concern and are not removed by upper-lid skin excision.

Bring details of eye surgery, contact-lens use, dryness, thyroid or neurological problems, and all medicines and supplements. The surgeon may assess lid position, brow support, eye closure, skin excess and the ocular surface before discussing an incision plan.

After surgery, swelling, bruising, tightness and temporary irritation can occur. Follow the prescribed wound and eye-care instructions, protect the area from rubbing and attend follow-up visits. Plasma pen treatment is different from surgery and should not be assumed to correct the same anatomy.

A suddenly drooping eyelid needs urgent medical assessment when it appears with double vision, unequal pupils, facial weakness, severe headache, confusion or weakness elsewhere. Those symptoms are not a routine cosmetic presentation.

After eyelid surgery, seek immediate advice for reduced vision, severe or escalating eye pain, marked one-sided swelling, persistent bleeding, inability to close the eye or worsening redness with discharge.

Ask which structure causes the droop, whether your eye surface needs treatment first, how much skin can be removed while preserving closure and which symptoms should trigger an urgent call after the procedure.
