Bleeding right after sex: what to know

The amount, whether penetration was painful, menopausal status, pregnancy possibility and accompanying discharge or pelvic pain all help guide the evaluation. Visible blood after sex cannot show its source or cause on its own.

Friction can cause a small surface injury, particularly when lubrication is limited. Genitourinary syndrome of menopause commonly presents with vaginal dryness, fragile tissue and discomfort that can be accompanied by bleeding after contact. Cervical inflammation, including from chlamydia, can present with bleeding after sex. Cervical dysplasia, cervical cancer and vaginal cancer may also present with postcoital bleeding, so ongoing symptoms should not be attributed to friction without examination.

Bleeding may coincide with the beginning or end of a period rather than arise from sex. If pregnancy is possible, pregnancy-related bleeding must also be considered.

Pause penetration if it is painful or bleeding continues. Note the colour and amount of blood, where you were in your cycle, whether dryness or pain occurred, and whether there is unusual discharge, odour or urinary discomfort. A water- or silicone-based lubricant may reduce friction during future sex, but it does not replace assessment for recurrent bleeding.

Book a clinician visit if bleeding happens again, has no clear minor injury, or occurs after menopause. The visit may involve a pregnancy test, infection testing, inspection of the vagina and cervix, and a check that cervical screening is up to date. Further tests depend on what is found.

Get urgent care if bleeding is heavy or is accompanied by fainting, severe dizziness, severe pelvic or abdominal pain, fever, or feeling very unwell. If pregnancy is possible, urgent symptoms include bleeding with one-sided pain, shoulder-tip pain, weakness or collapse. Seek prompt assessment after sexual injury if pain is severe, a tear seems deep or bleeding does not settle.

You might ask whether the blood appears to come from the vagina or cervix, whether infection testing is advisable, how dryness can be treated, and whether cervical screening or a specialist examination is needed. Mention medicines that affect bleeding and any possibility of pregnancy.
