Bleeding the day after sex

A small amount may become noticeable hours later because blood remains in the vagina before coming out. Timing alone cannot identify the cause, and bleeding that happens close to an expected period may also be menstrual.

Friction, limited lubrication, a small vaginal injury, hormonal dryness, cervical inflammation, or a cervical polyp can cause contact bleeding. Infections such as chlamydia may inflame the cervix and can occur without other obvious symptoms. Changes related to menopause can make vaginal tissue thinner and more prone to bleeding.

Cervical dysplasia, cervical cancer, and vaginal cancer can present with bleeding after sex, but the symptom alone does not establish any diagnosis. An examination is the practical way to locate the bleeding and decide whether infection testing, cervical screening, imaging, or another investigation is appropriate.

Note the amount and colour, whether bleeding followed penetrative sex, whether there was dryness or pain, and where you were in your menstrual cycle. Also record unusual discharge, pelvic discomfort, contraception, pregnancy possibility, and when your last cervical screening was performed.

Avoid further penetration until pain or active bleeding has settled. For future sex, more time for arousal and a compatible lubricant may reduce friction, but recurrent bleeding still needs assessment even if lubrication helps. Arrange sexual health testing if there has been possible exposure to an infection.

Seek urgent care for heavy bleeding, faintness, severe pelvic or abdominal pain, or bleeding with a positive pregnancy test. Fever, worsening pain, or feeling acutely unwell also warrants prompt medical attention.

Make a non-emergency appointment for any unexplained bleeding after sex, especially if it recurs, happens after menopause, or accompanies unusual discharge or pelvic pain. Keep cervical screening up to date, but remember that screening is not a diagnostic test for current symptoms; bleeding still requires its own clinical assessment.

Ask whether the bleeding appears to come from the vagina, cervix, or womb; whether infection and pregnancy tests are appropriate; and whether you need an examination or follow-up. Mention dryness, pain during sex, menstrual changes, medicines, and any previous abnormal cervical screening result.
