Blood in Urine After Sex

True blood in urine can occur with a urinary infection, stones, irritation or another urinary-tract problem. Blood from the vagina or cervix may instead become visible in the toilet after intercourse. Identifying the source helps direct the examination and tests.

Urinary bleeding may colour the urine throughout the stream and can accompany burning, urgency, frequent urination or pain in the side or back. Friction around the urethral opening can also cause a small amount of surface bleeding.

Vaginal bleeding after sex may follow dryness, inflammation, infection, cervical changes or a small injury. Genitourinary syndrome of menopause often presents with vaginal dryness and fragile tissues that can bleed with contact. Chlamydia can present with bleeding after sex, discharge or pelvic discomfort. Cervical dysplasia, cervical cancer and vaginal cancer can also present with contact bleeding, but the symptom alone does not identify the cause.

Arrange a medical appointment, particularly if the bleeding recurs or is clearly mixed with urine. Record whether blood appears before, during or after the urine stream and whether wiping shows vaginal blood. Note pain, urinary symptoms, discharge, missed periods and any new sexual partner.

A clinician may request a urine sample and pregnancy test, examine the vulva, vagina and cervix, or offer infection testing. Depending on the findings, further urinary or gynaecological assessment may follow. Avoid sex if it is painful or triggers fresh bleeding until you have advice, and do not use leftover antibiotics.

Seek urgent care for heavy bleeding, inability to pass urine, clots blocking the urine flow, severe pelvic or flank pain, fainting, marked weakness, fever with shaking chills, or possible pregnancy with pain or bleeding. These features can require prompt testing and treatment.

Request a timely review for any visible blood in urine, repeated bleeding after sex, bleeding after menopause, unusual discharge, pain during sex or persistent urinary symptoms. Bleeding that stops still deserves assessment when it has returned or its source is unclear.

Ask whether the blood appears urinary or vaginal, which samples or examinations can clarify the source, and when results will be available. If cervical changes are found, ask what they mean and whether observation, additional testing or a procedure such as cone biopsy is relevant to your specific diagnosis.
