What the blood may mean

Dry air, forceful nose blowing, allergies, a cold, or repeated coughing can damage small surface vessels. Blood from a nosebleed may drain backwards into the throat, then be cleared by coughing. Bright red streaks mixed with mucus can also come from inflamed airways, such as during Acute Bronchitis. Ongoing cough may be associated with Asthma, Bronchiectasis, or Chronic Cough; these possibilities cannot be diagnosed from blood alone.

Tell the clinician whether the blood was streaked through phlegm, came up without mucus, or was first seen in the nose or mouth. Also mention fever, chest pain, breathlessness, wheezing, smoking, recent injury, and medicines that affect clotting.

What to do now

Sit upright and lean slightly forward if the nose is bleeding. Pinch the soft part of the nose continuously, breathe through the mouth, and avoid swallowing blood. Do not tilt the head back or put tissues deep into the nostril. Once it settles, avoid forceful blowing and strenuous activity for a while. If coughing brings up blood, save a photo or note its colour and amount, and arrange medical advice rather than self-treating with leftover antibiotics.

When to seek care

Get emergency help for trouble breathing, chest pain, faintness, confusion, blue lips, vomiting blood, or bleeding that is heavy or does not stop with firm pressure. Seek same-day medical assessment for repeated blood when coughing, clots, fever with worsening illness, or a nosebleed after a significant injury. Children, older adults, and people taking blood thinners should use a low threshold for prompt assessment.
