Bleeding and Signs of Early Labour

A small amount of blood-streaked mucus can occur as the cervix changes, but fresh bleeding can also come from the cervix, placenta or another pregnancy complication. The colour or amount alone cannot confirm the cause, so contact maternity care rather than waiting for the bleeding to settle.

Preterm labour means the cervix begins opening with labour contractions before the pregnancy reaches full term. Possible features include regular tightenings, period-like cramps, low backache, pelvic pressure, a gush or trickle of fluid, and a change in vaginal discharge. Bleeding may accompany cervical change, but it does not establish that labour has begun.

Assessment may include questions about contractions and fluid loss, checks of your pulse and blood pressure, monitoring of the baby's heartbeat, and examination or imaging when appropriate. Your maternity team will tailor testing to the stage of pregnancy and your symptoms.

Call your maternity unit now and describe the bleeding, pain, tightenings, fluid loss and your baby's movements. Use a sanitary pad so you can observe the blood; do not insert a tampon or have penetrative sex. Bring your pregnancy records and medicine list if you are asked to attend.

If contractions are occurring, note when each starts and stops without delaying the call. Treatment depends on the cause, gestational stage and examination findings; it may include monitoring, medicine to support the baby's development, or other pregnancy care.

Call emergency services for heavy or rapidly increasing bleeding, severe constant abdominal pain, fainting, breathing difficulty, or feeling acutely unwell. Seek immediate maternity assessment for reduced or absent fetal movement, a gush of fluid, strong regular contractions, or pressure suggesting the baby may be coming.

Ask whether the cervix is changing, whether the membranes appear intact, and how the baby's heartbeat looks. Clarify what monitoring or treatment is recommended, what changes should trigger another urgent call, and where you should return if symptoms recur.
