Bleeding during sex with an IUD

Bleeding during or after sex with an IUD deserves assessment if it persists, recurs, or comes with pain. Light spotting may follow friction or irritation, but bleeding can also come from the cervix or vagina and may be unrelated to the device.

A dry or irritated vaginal lining can bleed with contact. Cervical inflammation, a cervical polyp, or an infection can also cause bleeding during or shortly after sex. Pelvic inflammatory disease may present with pelvic pain, unusual discharge, fever, or bleeding between periods.

An IUD can cause changes in menstrual bleeding, especially after insertion, but contact bleeding should not automatically be attributed to it. New deep pain, strings that feel noticeably different, or being unable to feel strings that were previously easy to find can prompt a check of the device's position. Do not pull on the strings.

Pause penetrative sex if it is painful or bleeding continues. Note when the bleeding began, its colour and amount, whether it happens every time, and any pelvic pain, discharge, odour, urinary symptoms, fever, or change in IUD strings. A water-based lubricant may reduce friction if dryness is contributing.

Arrange a clinical assessment for repeated bleeding. The clinician may examine the vagina and cervix, offer infection testing, confirm that cervical screening is up to date, and check IUD position when symptoms or the string examination raise concern. Take a pregnancy test if your period is late or pregnancy is possible, even if you use an IUD.

Seek urgent care for heavy bleeding, severe or worsening pelvic pain, fainting, marked dizziness, shoulder-tip pain, fever, or a positive pregnancy test with pain or bleeding. These features need prompt evaluation, including assessment for pregnancy outside the uterus.

Book a routine appointment if contact bleeding returns, continues, occurs with unusual discharge, or starts after sex had previously been comfortable and free of bleeding. Avoid relying on string position alone to explain symptoms.

Ask whether the cervix or vaginal tissue looks irritated, whether infection testing is appropriate, and whether the IUD strings or symptoms suggest a position check. You can also ask what bleeding pattern to expect from your specific IUD and what change should bring you back.
