Bleeding during sex after IUD insertion

Insertion can temporarily cause spotting and cramp-like discomfort. Sex may simply coincide with this bleeding rather than cause it, yet bleeding specifically associated with penetration can also come from irritated vaginal or cervical tissue and should not automatically be attributed to the device.

Bleeding soon after fitting may reflect the insertion procedure or the change in uterine bleeding caused by the IUD. Other possibilities include dryness, a cervical change, or an infection. An IUD that has shifted may be considered when bleeding accompanies new pelvic pain, altered thread length, or an inability to feel threads that were previously present.

A clinician can distinguish these possibilities by asking when the device was fitted, which type it is, and whether bleeding occurs at other times. An examination, pregnancy test, infection test, or device-position check may be offered according to your symptoms.

Pause sex if penetration is uncomfortable or causes fresh bleeding. Track the flow and any cramps, discharge, odour, fever, or thread change. You can check the threads only as your fitting service taught you; do not pull them or try to reposition the IUD.

Contact the clinic that fitted the device if symptoms are continuing or you are unsure about its position. Until position and pregnancy protection are confirmed, follow the clinic's advice about backup contraception. Testing for sexually transmitted infections may be relevant when exposure is possible.

Get urgent help for severe or worsening lower-abdominal pain, heavy bleeding, fainting, fever, unusual or foul-smelling discharge, a positive pregnancy test, or bleeding with shoulder-tip pain. These features need timely evaluation for infection, pregnancy complications, or device displacement.

Book a routine but prompt check for repeated bleeding with sex even if it is light. Examination can identify cervical or vaginal irritation and confirm whether an IUD assessment is also needed.

Ask whether the bleeding pattern fits your IUD type and insertion timing, whether the threads or device position should be checked, and whether you need a pregnancy or infection test. Clarify what contraception to use while awaiting assessment and which symptoms warrant same-day care.
