Bleeding during sex after a hysterectomy

A small blood-stained discharge can occur as healing tissues are disturbed, but intercourse may need to pause until the surgical area has healed and bleeding or discharge has stopped. Follow the recovery instructions from your own surgeon, because timing differs with the operation and healing progress.

Early after surgery, the top of the vagina may still contain healing tissue or stitches, and penetration can irritate that area. Later bleeding may relate to fragile scar tissue, vaginal dryness, infection, or another vaginal or cervical source if the cervix was retained. The operation performed and whether the cervix or ovaries were removed help narrow the possibilities.

Bleeding alone does not identify the cause. Its colour, amount, timing, associated pain, odour, and whether it happens again all help a clinician decide what examination is appropriate.

Stop penetration if it causes fresh bleeding or pain. Use a pad rather than inserting a tampon so you can observe the flow, and note whether there are clots, discharge, pelvic pressure, or fever. Contact the surgical team if you are still within the postoperative recovery period; otherwise arrange a gynaecology or primary-care assessment.

Do not resume penetrative sex until your clinician's healing advice has been met and the bleeding has settled. When cleared to restart, move gently and consider a water-based lubricant if dryness is present.

Seek urgent medical help for heavy or rapidly increasing bleeding, large clots, severe or worsening pelvic or abdominal pain, faintness, breathing difficulty, fever, or foul-smelling discharge. These symptoms can signal significant blood loss, infection, or a problem at the healing surgical site.

Arrange a prompt appointment for lighter bleeding that recurs with sex, starts after recovery seemed complete, or is paired with persistent discomfort. The clinician may examine the vagina and surgical scar and review the operation record.

Ask which organs were removed, whether the cervix remains, and whether the vaginal cuff was fully healed at your last examination. Other useful questions include whether dryness or scar tissue could explain the bleeding, whether infection testing is needed, and what signs should change the urgency of your care.
