Bleeding During Sex After a Long Break

A long break from sexual activity does not by itself cause bleeding or create lasting vaginal tightness. Discomfort, tissue dryness, or a surface tear may be involved, and an examination can check for other sources.

Low arousal, limited lubrication, rapid penetration, or pelvic-floor tightening can increase friction. Tissue may also be dry or fragile during breastfeeding, around menopause, or with some medicines. Genitourinary syndrome of menopause can present with vaginal dryness, burning, pain during sex, and contact bleeding. Cervical inflammation linked to chlamydia can present with postcoital bleeding. Cervical dysplasia, cervical cancer, and vaginal cancer are other possible presentations, so persistent bleeding should not be attributed to the gap alone.

Stop if penetration hurts or bleeding begins, and allow soreness to settle before trying again. More time for arousal, slower penetration, clear communication, and a suitable lubricant may reduce friction. Avoid fragranced products on irritated genital tissue. Keep track of how much you bleed, how long it continues, whether it occurs every time, and any dryness, discharge, pelvic pain, urinary symptoms, or bleeding at other times. Arrange an appointment if it returns or lacks an obvious surface cause.

Emergency care is appropriate for heavy bleeding, severe pelvic or abdominal pain, fainting, pronounced dizziness, weakness, or possible pregnancy with bleeding and one-sided pain. Fever with pelvic pain or feeling acutely unwell also needs prompt help. Bleeding after menopause, recurring bleeding during sex, unusual discharge, or ongoing pain calls for a timely clinical appointment, even if each episode is light.

Ask whether dryness, infection, cervical changes, or another finding could explain the bleeding. Clarify whether a vaginal and cervical examination, infection tests, pregnancy testing, or cervical screening is suitable. If dryness is ongoing, discuss moisturisers, lubricants, and whether a prescribed local treatment fits your health history.
