Bleeding with blood clots after sex

A clot forms when blood collects before leaving the vagina. It does not identify whether bleeding came from a small vaginal injury, the cervix or the womb.

Friction, dryness or a small tear can cause fresh bleeding during or after penetration. Vaginal tissue may become thinner and more fragile around and after menopause. Cervical inflammation, infection or cervical cell changes can also present with bleeding after sex. Blood already present from a period may be noticed after intercourse.

Clots may appear when the flow is enough for blood to pool, but their size or colour cannot diagnose a cause. Recurrent postcoital bleeding warrants examination even when each episode settles.

Pause penetration while active bleeding or soreness continues. Use a pad to observe the amount, and write down when bleeding began, whether it followed pain or dryness, how many pads were needed, and whether clots continued. Avoid inserting tampons or menstrual cups until the source of significant bleeding is clearer.

Arrange an appointment with a gynaecologist, primary care clinician or sexual health service. Assessment may include a pregnancy test, infection testing and examination of the vagina and cervix. Mention overdue cervical screening, a new sexual partner, menopausal status and any blood-thinning medicine.

Go to emergency care for bleeding that is heavy or rapidly increasing, fainting, severe dizziness, weakness, severe pelvic or abdominal pain, or bleeding after a possible injury that may need repair. If pregnancy is possible, bleeding with one-sided pain, shoulder-tip pain or collapse requires immediate assessment.

Request prompt review if bleeding recurs after sex, occurs after menopause, comes with foul-smelling discharge or fever, or continues beyond the sexual contact. Urgency is based on the bleeding and associated symptoms, not on guessing the diagnosis.

Ask whether the blood appears to come from the vaginal wall, cervix or womb; whether infection or pregnancy testing is needed; and how your cervical screening history affects the assessment. Also ask what to use for dryness if fragile tissue is suspected.
