Bleeding between periods while taking the pill

It may reflect the hormonal effect on the womb lining, especially after starting or changing pills. Missed doses, late doses, vomiting, diarrhoea or interacting medicines can also disrupt bleeding and may affect contraceptive protection.

The pill keeps the womb lining thin through hormone exposure. That lining can shed unpredictably while the body adjusts or when hormone levels fluctuate. Bleeding can range from a smear seen when wiping to a flow that resembles a period.

The pill is not the only possible explanation. Pregnancy, infection, cervical irritation, polyps or fibroids can also present with bleeding between periods. The timing, pill type, sexual health history and accompanying symptoms help a clinician narrow the cause.

Keep taking the pill according to its packet instructions unless a clinician tells you to change it. Check the leaflet for advice specific to missed or late pills because combined and progestogen-only pills have different rules. If vomiting, diarrhoea or another medicine could have reduced absorption or effectiveness, ask a pharmacist or clinician whether backup contraception or emergency contraception is needed.

Record bleeding days, flow, pain, missed tablets and sex without barrier protection. Arrange a pregnancy test when pregnancy is possible, and book a review if the pattern persists, begins after previously settled cycles or follows sex.

Seek urgent care for heavy bleeding, fainting, pronounced dizziness, severe pelvic or abdominal pain, shoulder-tip pain, fever, or a positive pregnancy test with bleeding. Sudden chest pain, difficulty breathing, coughing blood, or painful swelling in one leg also requires emergency help while taking a combined hormonal pill.

Arrange a timely appointment for repeated bleeding after sex, unusual discharge, pelvic pain, pain during sex, or bleeding that continues despite correct pill use.

Bring the pill packet or its exact name. Ask whether your dosing pattern could explain the bleeding, whether pregnancy or infection testing is appropriate, and whether another formulation or contraceptive method may give better bleeding control for you.
