Bleeding before your period after intercourse

The blood may be triggered by friction, begin because a period is approaching, or arise from inflammation, infection, a cervical change, a polyp or another source in the reproductive tract. The fact that it occurred near an expected period does not prove that it is menstrual blood.

Fresh red blood immediately after sex can follow a small vaginal surface injury or contact with a sensitive cervix. Dryness, vigorous penetration and insufficient lubrication make friction more relevant. Cervical inflammation, including that caused by chlamydia, may present with bleeding after sex, unusual discharge or pelvic discomfort.

Endometrial polyps and uterine fibroids may present with bleeding between periods. Cervical dysplasia and cervical cancer can present with contact bleeding, while womb cancer can present with abnormal vaginal bleeding; these symptoms have many non-cancerous explanations, and an examination is needed to determine the source.

Note whether blood appeared during sex, on wiping afterward or continued into the next day. Record flow, colour, cycle date, pain, discharge, contraception, missed tablets and whether the next period arrived as expected. A pregnancy test is sensible if pregnancy could have occurred or the period is late.

Avoid penetration until soreness or active bleeding settles. A water-based lubricant may reduce friction next time, but it is not a substitute for assessment when bleeding recurs. Arrange a clinician visit for examination and, when relevant, infection tests, cervical screening review or pelvic imaging.

Seek urgent help if bleeding becomes heavy, you have severe or escalating pelvic pain, you faint or feel markedly dizzy, or you develop fever and feel unwell. If pregnancy is possible, bleeding with one-sided pelvic pain, shoulder-tip pain or faintness needs immediate assessment.

Repeated bleeding after intercourse, bleeding after menopause, persistent unusual discharge, or bleeding with pain during sex should be reviewed promptly even when the amount is small.

Ask whether the blood appears to come from the vagina, cervix or uterus, and whether infection testing or a pregnancy test is indicated. Confirm when your next cervical screening is due and ask whether a polyp, fibroid or other structural cause needs imaging or specialist review.
