Bleeding and discharge after sex

Friction, vaginal dryness, inflammation of the cervix or vagina, and sexually transmitted infections can cause these symptoms. Changes on the cervix or vagina are less common possibilities that an examination can help identify.

Dryness or small surface tears may cause fresh blood immediately after penetration, especially around menopause, during breastfeeding or when arousal and lubrication were limited. Genitourinary syndrome of menopause can present with dryness, burning, discomfort during sex and spotting.

Yellow, green, frothy, thick or unpleasant-smelling discharge can accompany infection, while pelvic pain, pain during sex or urinary burning adds useful diagnostic information. Chlamydia may present with discharge and bleeding after sex, though some people notice no symptoms. Cervical dysplasia, cervical cancer and vaginal cancer can also present with bleeding after sex; the symptom itself does not determine the diagnosis.

A clinician may ask about the discharge, cycle timing, contraception, pregnancy possibility, new sexual partners and cervical screening history. Depending on the answers, they may examine the vulva, vagina and cervix, take infection swabs, perform a pregnancy test, or arrange cervical testing or further evaluation.

Pause sex if it causes pain or fresh bleeding, and avoid vaginal douching or fragranced internal products because they can worsen irritation and alter discharge. Arrange an appointment, and consider infection testing when there is a new partner or possible exposure. Until infection has been assessed, barrier protection helps reduce onward transmission.

Write down the colour, consistency and smell of discharge and whether bleeding happens every time or only after deeper penetration. If dryness seems relevant, a water-based lubricant may reduce friction, but recurring bleeding still needs evaluation.

Get urgent help if bleeding is heavy, pain is severe, you feel faint or very unwell, or pregnancy is possible and bleeding accompanies pelvic or shoulder-tip pain. Fever with worsening lower abdominal pain and abnormal discharge also needs urgent assessment.

Arrange prompt review for any bleeding after menopause or for repeated bleeding after sex, even without pain. These patterns need examination rather than self-diagnosis.

Ask whether the cervix and vaginal tissues look irritated, whether infection tests are needed, and whether your cervical screening is up to date. If menopause-related dryness is suspected, ask which non-hormonal or hormonal vaginal treatments suit your history.
