Bleeding After Sex With an IUD

Spotting may coincide with sex without being caused by the device. Friction, vaginal dryness, cervical inflammation, infection, or a cervical growth can also cause fresh contact bleeding.

Bleeding that occurs at unrelated times may fit a broader change in menstrual pattern, especially after an IUD is fitted. Bleeding that reliably starts with penetration points toward contact with sensitive tissue, although symptoms cannot locate the source with certainty.

New pelvic pain, unusual discharge, fever, pain during sex, or a noticeable change in the IUD threads needs clinical attention. Cervical inflammation associated with chlamydia can present with contact bleeding. Cervical dysplasia and cancers of the cervix or vagina can also present this way, while many episodes have non-cancerous explanations.

Record when the IUD was inserted, the type if known, your usual bleeding pattern, and whether this episode involved pain or discharge. If you were taught to check the threads, notice only whether they feel different; do not pull them or try to reposition the device.

Use a pad or liner to monitor bleeding and avoid penetration while pain or active bleeding continues. Arrange an appointment for recurrent contact bleeding, persistent spotting that concerns you, new deep pain during sex, or altered threads. A clinician can check the cervix, assess device position when indicated, test for infection, and review cervical screening.

Seek urgent assessment for heavy bleeding, severe or one-sided pelvic pain, fainting, pronounced dizziness, fever, or bleeding with a positive pregnancy test or possible pregnancy. Pregnancy symptoms with an IUD in place require timely evaluation.

Contact a healthcare service promptly if the device appears to be coming out, hard plastic can be felt, the threads suddenly seem very different, or pain and discharge develop after insertion. Use alternative contraception until a clinician advises whether the IUD remains correctly placed and effective.

Ask whether the bleeding pattern fits the type and timing of your IUD, whether its position should be checked, and whether pregnancy or infection testing is appropriate. Clarify what thread changes to watch for and when penetrative sex or your usual contraception plan can safely continue.
