Bleeding after sex following pregnancy

After birth, normal uterine bleeding and discharge can overlap with blood noticed after sex, making the source hard to identify. Vaginal or perineal tears, an episiotomy site and other tissues may still be healing. Hormonal changes, particularly while breastfeeding, can reduce lubrication and make the vaginal lining easier to graze.

Bleeding can also arise from cervical irritation, a polyp or an infection. Chlamydia may present with bleeding after sex, although symptoms alone cannot diagnose it. If pregnancy ended through miscarriage or termination rather than birth, tell the clinician because the expected recovery pattern and checks may differ.

Pause sex if penetration hurts or restarts bleeding. Use a pad, not a tampon, to observe the flow and record whether it is becoming lighter or heavier. If your maternity team advised pelvic rest or you have an unhealed tear, follow that guidance until reassessment. Once healing is confirmed, going slowly and using a water-based lubricant can reduce friction.

Book an appointment for repeated spotting, pain with sex, unusual discharge, bleeding that had stopped and then returned, or concern about wound healing. The review may include an examination of healing sites and the cervix, along with infection testing when indicated. If you are currently pregnant again, report any vaginal bleeding to your maternity service rather than assuming sex caused it.

Get urgent help for bleeding that is heavy, rapidly increasing or accompanied by large clots, faintness, weakness, shortness of breath or severe abdominal or pelvic pain. Fever, chills, worsening tenderness or bad-smelling discharge can signal infection and require prompt care. Sudden chest pain, difficulty breathing or collapse is an emergency regardless of the amount of vaginal bleeding.

Describe how the pregnancy ended, how long ago it was, whether postpartum discharge had stopped, and any tears or procedures you had. Ask whether healing is complete, whether dryness or infection is visible, whether testing is needed, and what signs should change the urgency of your care.
