Bleeding after sex following a hysterectomy

If surgery was recent, friction may disturb healing tissue or stitches at the top of the vagina. Penetration should wait until the surgeon's advised recovery period has passed, wounds have healed and discharge has stopped. Months or years later, dryness, delicate vaginal tissue, a small tear, inflammation or infection can produce contact bleeding. Removal of the ovaries can contribute to dryness through lower oestrogen levels.

The source also depends on the operation. A total hysterectomy removes the uterus and cervix, while some procedures leave the cervix in place. That distinction helps a clinician decide whether to inspect the vaginal cuff, vagina or remaining cervix.

Stop penetration if it causes bleeding or pain. Note the colour and amount, whether it happened once or repeatedly, and any discharge, pelvic discomfort or urinary symptoms. Arrange a clinical review rather than repeatedly testing whether sex triggers it. A clinician may examine the vagina and surgical scar, clarify whether the cervix remains, and offer infection testing when relevant.

For mild dryness after healing is confirmed, a water-based lubricant and more time for arousal may reduce friction. Avoid vaginal douching and scented products because they can irritate tissue. Ask whether a vaginal moisturiser or prescribed local treatment suits your surgical and medical history.

Seek urgent medical care for heavy or increasing bleeding, clots, severe pelvic or abdominal pain, fainting, marked dizziness, fever, or foul-smelling discharge. Contact the surgical team promptly if bleeding begins during the recovery period or sex occurred before you were cleared for penetration. Even light bleeding deserves a routine appointment when it recurs or appears after a long symptom-free interval.

Ask which type of hysterectomy you had, whether the vaginal cuff has fully healed, and whether the cervix or ovaries remain. You can also ask whether the examination suggests dryness, scar tissue, infection or another cause, what treatment fits the findings, and when penetration can be resumed.
