Bleeding after sex with abdominal pain

Friction or a small vaginal injury may cause surface bleeding and local soreness, while deeper lower-abdominal pain points to a need for broader assessment. Cervical inflammation related to an infection such as chlamydia can present with contact bleeding and pelvic discomfort. Pain may also arise from a separate pelvic or abdominal problem that happened around the same time as sex.

Cervical cancer and vaginal cancer can present with bleeding after sex, and cervical dysplasia can be associated with contact bleeding. These are possibilities a clinician considers alongside more common explanations; the symptom combination cannot identify a diagnosis on its own.

Stop penetrative sex and note where the pain sits, whether it is constant or comes in waves, and whether movement makes it worse. Record the bleeding amount, last period, pregnancy possibility, contraception, discharge, fever, nausea, urinary or bowel symptoms, and any recent new sexual partner. Do not insert a tampon while active bleeding and pain are unexplained.

Contact a healthcare service for advice on the appropriate timing of assessment. The clinician may check the abdomen and pelvis, examine the vagina and cervix, and offer pregnancy or infection testing according to your circumstances. Treatment depends on the source rather than the bleeding alone.

Seek urgent medical care now for severe or rapidly worsening pain, heavy bleeding, fainting, marked dizziness, shoulder-tip pain, difficulty breathing, or a rigid or very tender abdomen. Bleeding with a missed period or positive pregnancy test requires urgent assessment, particularly when pain is one-sided. Fever, vomiting, feeling acutely unwell, or offensive-smelling discharge also calls for prompt care.
