Bleeding after sex that looks like a period

Sex may coincide with the start of menstruation, but period-like flow can also come from a vaginal injury, the cervix, the uterus or a pregnancy-related problem. Do not rely on timing alone if the bleeding differs from your usual period.

A vaginal tear may follow forceful penetration, friction or dryness. Cervical inflammation, a polyp or chlamydia may bleed when touched. Hormonal changes can also cause uterine bleeding that happens to begin around sex.

Cervical dysplasia, cervical cancer and vaginal cancer can present with bleeding after sex, although the symptom has many other causes. If pregnancy is possible, bleeding with pelvic or abdominal pain requires prompt assessment because complications of pregnancy must be considered.

Stop penetration and use a sanitary pad so you can observe the flow. Note when it started, whether it is increasing, the presence of clots, and any pain, dizziness, discharge or fever. If pregnancy is possible, take a pregnancy test but do not delay urgent help for concerning symptoms.

Contact a healthcare professional if the flow is not your expected period, is heavier than your usual menstrual bleeding, or repeatedly follows sex. Evaluation may include a pelvic examination, pregnancy testing, infection testing and a review of cervical screening.

Seek emergency care if bleeding is heavy or rapidly increasing, you are passing large clots, you have severe abdominal or pelvic pain, or you feel weak, faint or very dizzy. These signs can accompany meaningful blood loss or another urgent condition.

Prompt care is also needed for bleeding with possible pregnancy, fever, foul-smelling discharge, or worsening pain. Any bleeding after menopause requires assessment, even if it resembles a period that later stops.

Describe how this flow compares with your usual menstruation and whether sex appears to trigger it. Ask whether the blood is vaginal, cervical or uterine in origin, whether pregnancy and infection have been excluded, and what follow-up is needed if it happens again.
