Bleeding After Sex in the First Trimester

A few spots that stop may reflect surface contact and do not by themselves show what is happening with the pregnancy. The amount, persistence, pain, and other symptoms determine how quickly you need assessment.

Early pregnancy increases blood flow to the cervix, making its surface more prone to spotting after penetration. Friction, vaginal irritation, a cervical polyp, or inflammation from an infection can also cause contact bleeding. Sex does not reach the pregnancy inside the uterus.

Not every episode comes from sexual contact even when the timing suggests it. First-trimester bleeding can also occur with early pregnancy loss or ectopic pregnancy. These conditions may present with bleeding and pain, so symptoms and clinical assessment matter more than the apparent trigger.

Use a pad or pantyliner rather than a tampon so you can monitor the blood. Note whether it is pink, red, or brown; whether it is only visible when wiping; and whether it is becoming heavier. Record cramps, one-sided pain, clots, fluid, fever, dizziness, and shoulder-tip pain.

Contact your obstetrician, maternity service, or early-pregnancy clinic and describe the episode, even if it has stopped. They can decide whether you need an examination, ultrasound, blood tests, or infection testing. Until you receive advice and the bleeding has settled, avoid further penetration.

Go to an emergency department or call local emergency services if bleeding is heavy or rapidly increasing, pain is severe or persistent, you feel faint or very dizzy, or you develop shoulder-tip pain. Prompt assessment is also needed for one-sided pelvic pain, fever, foul-smelling discharge, passage of tissue, or fluid from the vagina. Do not wait for a routine appointment if you feel acutely unwell.

Tell the clinician the first day of your last menstrual period, any ultrasound results, your blood group if known, and exactly when the bleeding began. Useful questions include where the blood appears to be coming from, whether the pregnancy location has been confirmed, what tests are appropriate, and which changes should bring you back immediately.
