Bleeding after sex: causes and next steps

A smear on tissue and heavier vaginal bleeding need different responses, but colour alone cannot reveal the cause. Note the amount, timing, pain, discharge, pregnancy possibility, and whether the bleeding has happened before.

Friction can make a small graze at the vaginal entrance, especially when lubrication is limited. Lower oestrogen levels can make vaginal tissue thinner and drier, including after menopause, during breastfeeding, or with some medicines. The cervix can bleed after contact when surface cells are delicate or a benign polyp is present.

Chlamydia and other infections may inflame the cervix and present with bleeding after sex, sometimes with discharge or pelvic discomfort. Cervical dysplasia, cervical cancer, and vaginal cancer can also present this way, although bleeding after sex has many other causes. The symptom should prompt evaluation rather than a conclusion about cancer.

A clinician will usually ask about periods, contraception, menopause, pregnancy possibility, pain, discharge, medicines, sexual health, and cervical screening. With your consent, they may examine the vulva, vagina, and cervix. A pregnancy test, urine or swab testing for infection, or further gynaecology assessment may be offered according to the findings.

Until soreness settles, avoid activities that trigger more bleeding. If dryness or friction seems relevant, allow more time for arousal and use a compatible lubricant. These measures can reduce irritation but do not replace evaluation when bleeding is recurrent, unexplained, or occurs after menopause.

Seek urgent care for bleeding that is heavy or rapidly increasing, severe pelvic or abdominal pain, fainting, pronounced dizziness, fever, or feeling seriously unwell. If a period is late or a pregnancy test is positive, bleeding with one-sided pain, shoulder-tip pain, weakness, or dizziness needs urgent evaluation.

Book a prompt non-emergency appointment for any postmenopausal bleeding or repeated bleeding after sex. Arrange sexual health testing if exposure is possible, particularly with new discharge, pain during urination, pelvic pain, or a new sexual partner.

Ask what the examination suggests about the source, whether infection and pregnancy tests are indicated, and whether cervical screening is due. If dryness is present, discuss whether a moisturiser, lubricant, or prescribed local treatment is suitable for your health history.
