Bleeding after sex when you have endometriosis

Bleeding may instead arise from vaginal friction, the cervix, an infection, a polyp, hormonal contraception, or bleeding between periods that happens to follow sex. The source matters because treatment differs.

Endometriosis is tissue similar to the womb lining growing outside the womb. It can cause deep pelvic pain with penetration, pain after sex, painful periods, bowel or bladder symptoms around menstruation, and difficulty conceiving. These symptoms can flare in patterns linked to the menstrual cycle.

Visible blood from the vagina immediately after contact points to a possible vaginal or cervical source that needs separate consideration. Chlamydia can present with bleeding after sex, as can cervical dysplasia and, less commonly, cervical or vaginal cancer. Having endometriosis does not protect against or establish any of these conditions.

Keep a brief record of bleeding in relation to sex and periods. Include whether penetration was painful at the entrance or deep in the pelvis, how long discomfort lasted, and whether you noticed discharge, bowel pain, urinary symptoms, or bleeding at other times.

Arrange a clinician or sexual health appointment if the bleeding returns or has no clear explanation. Depending on your circumstances, the visit may involve pregnancy testing, infection tests, a speculum examination of the vagina and cervix, and a check of cervical screening history. Treatment should target the identified cause rather than endometriosis by default.

Get urgent help if bleeding is heavy, you feel faint or markedly dizzy, pelvic pain is severe or escalating, or you develop fever or feel acutely unwell. A missed period or positive pregnancy test together with bleeding and one-sided pelvic pain, shoulder-tip pain, weakness, or dizziness requires urgent assessment.

Request timely review if bleeding keeps occurring after sex, appears between periods, develops after menopause, or accompanies persistent discharge or pelvic pain. New bleeding should not be dismissed simply because pelvic pain is familiar.

You might ask whether the bleeding seems vaginal, cervical, or uterine; whether infection testing or a pregnancy test is needed; and whether the pain pattern suggests an endometriosis flare. Also ask how any proposed hormonal treatment could affect bleeding patterns and when follow-up should occur.
