Bleeding After Sex During the Fertile Window

The timing alone cannot identify the source. Notice whether the blood appeared immediately after penetration, mixed with fertile cervical mucus, continued afterward, or returned after later sexual contact.

Around ovulation, a small amount of mid-cycle spotting may coincide with sex. Blood that begins directly after penetration may instead come from delicate tissue on the cervix, a cervical polyp, friction, or a small vaginal surface injury. Dryness can occur at any cycle stage, especially when arousal or lubrication is limited.

An infection such as chlamydia can inflame the cervix and may present with bleeding after sex, unusual discharge, pelvic discomfort, or pain when passing urine, although infection can also cause few symptoms. Cervical dysplasia and cervical cancer can also present with contact bleeding, so persistent bleeding deserves an examination rather than an assumption based on cycle timing.

Pause penetration until active bleeding and soreness have settled. If you resume sex, use generous water-based lubricant and stop if pain or fresh bleeding returns. Record the cycle day, colour and amount of blood, whether it followed penetration, and any discharge, odour, urinary symptoms, fever, or pelvic pain.

Arrange a gynaecology or sexual-health appointment if this is unexplained, happens again, or comes with other symptoms. Assessment may include a pregnancy test, infection testing, and examination of the vagina and cervix. If pregnancy is possible and your next period is late, take a home pregnancy test at the appropriate time stated in its instructions.

Seek urgent medical care for heavy or increasing bleeding, severe pelvic or abdominal pain, fainting, marked dizziness, weakness, fever, or foul-smelling discharge. If a pregnancy test is positive, or your period is late and pregnancy is possible, bleeding with one-sided pelvic pain, shoulder-tip pain, dizziness, or fainting needs emergency assessment because these can be warning signs of an ectopic pregnancy.

Ask whether the bleeding appears to come from the cervix, vagina, or uterus; whether you need a pregnancy test or infection swab; whether your cervical screening is due; and what follow-up is needed if the examination is normal but bleeding returns. Mention new contraception, medicines, recent procedures, and any possibility of pregnancy.
