Dark-Brown Bleeding After Sex: What It Can Mean

Brown colour develops when blood has had time to oxidise before leaving the vagina. It may be a small amount of menstrual blood or spotting that appears after penetration, yet colour alone cannot show whether the blood came from the vagina, cervix, or uterus.

Possible explanations include friction or a small vaginal tear, vaginal dryness, a cervical polyp, or cervical ectropion, in which delicate cells on the outer cervix bleed easily when touched. Reduced oestrogen around or after menopause can make vaginal tissue thinner and more prone to injury. Hormonal contraception can also cause irregular spotting.

An infection such as chlamydia may inflame the cervix and can present with bleeding after sex, sometimes with unusual discharge, pelvic discomfort, or pain when urinating. Cervical dysplasia may cause no symptoms, while cervical or vaginal cancer can present with abnormal bleeding. These possibilities are reasons for examination, not conclusions that can be drawn from brown blood.

Arrange an appointment with a gynaecologist, primary care clinician, or sexual health clinic, even if the spotting was light or stopped. Note when it happened, the amount and colour, whether it followed painful or dry sex, the date of your last period, medicines and contraception, and any discharge or pelvic symptoms.

Assessment may include a pregnancy test, an examination of the vagina and cervix, and a urine sample or vaginal swab for sexually transmitted infections. Depending on the findings and your screening history, the clinician may discuss cervical screening, colposcopy, ultrasound, or another investigation. Avoid penetrative sex if it restarts the bleeding or causes pain; a water-based lubricant may reduce friction when dryness is the likely issue, but it does not replace assessment.

Go to an emergency department for heavy or rapidly increasing bleeding, severe lower abdominal or pelvic pain, fainting, marked dizziness, weakness, shortness of breath, or shoulder-tip pain. If pregnancy is possible, bleeding together with pelvic or abdominal pain, dizziness, or fainting needs urgent assessment because an ectopic pregnancy must be excluded.

Seek prompt medical care for fever, foul-smelling discharge, worsening pelvic pain, or feeling acutely unwell. Any vaginal bleeding after menopause should be evaluated promptly, including a single episode of brown spotting after sex. Recurrent bleeding, bleeding between periods, or new bleeding after sex also warrants an appointment even without pain.

Ask where the bleeding appears to come from, whether the cervix shows ectropion, a polyp, inflammation, or another change, and whether infection testing is appropriate. You can also ask if your contraception, menopause-related dryness, pregnancy status, or cervical screening history changes the recommended tests.

If a cervical abnormality is found, ask what the result means, whether colposcopy or biopsy is needed, how and when results will be shared, and which symptoms should lead you to seek care sooner.
