Could a cyst explain bleeding after sex?

The cyst's location and the presence of a visible lump, tenderness, deep pelvic pain or discharge help separate these possibilities.

A blocked gland near the vaginal entrance can form a lump that may or may not be tender. Friction may irritate the overlying skin, particularly if the area is swollen, infected or has opened and started draining.

Ovarian cysts lie deeper in the pelvis. They can be associated with pressure, one-sided pain, bloating or pain during sex, but bleeding seen immediately after penetration may have a separate vaginal or cervical source.

Cervical inflammation, dryness, polyps and cervical cell changes can produce contact bleeding. A presumed cyst should not delay examination when blood repeatedly follows sex.

Avoid squeezing, piercing or draining a lump yourself. Pause activities that rub the area, keep it clean and dry, and note any change in size, redness, warmth, drainage or pain.

Record whether blood comes from the lump, vaginal canal or is mixed with discharge if you can do so without inserting anything. Also track pelvic pain, cycle timing and whether bleeding occurs without sex.

Arrange an examination for a new genital lump, a lump that persists or returns, or post-sex bleeding that recurs. The clinician may inspect the skin and cervix and use pelvic examination or imaging when a deeper cyst is suspected.

Get urgent help for sudden severe pelvic pain, fainting, heavy bleeding, vomiting with intense pain or a possible pregnancy with pain and bleeding. These symptoms require assessment beyond checking a surface cyst.

Seek prompt review if a lump becomes very painful, rapidly enlarges, drains pus, is accompanied by fever, or if genital bleeding follows menopause.

Ask where the cyst is located, whether it could account for the visible blood, and whether the vagina and cervix should also be examined. If pain is deep or one-sided, ask whether pelvic imaging is appropriate.
