Bleeding After Sex After Delivery

The timing, amount, colour, associated pain, and whether normal post-birth discharge had already stopped all help distinguish surface irritation from a postpartum problem.

After a vaginal birth, tears, stitches, and the tissues around the vaginal opening may still be healing. Friction can disturb a tender area. Hormonal changes, especially while breastfeeding, can reduce vaginal lubrication and make small surface cracks more likely. The cervix may also bleed when touched if it is inflamed.

Post-birth bleeding and discharge can continue while the uterus recovers, so blood noticed after sex may not all come from intercourse. Infection is more concerning when bleeding occurs with fever, worsening pelvic pain, unpleasant-smelling discharge, or feeling unwell. An examination can identify the source rather than relying on colour alone.

Pause penetrative sex while bleeding or soreness continues. Use a sanitary pad rather than an internal menstrual product so you can observe the flow. Note whether the blood appeared only on wiping, whether it continued afterwards, and whether there was pain, discharge, or a change in your usual postpartum bleeding.

Arrange a maternity or gynaecology review if bleeding returns with intercourse, healing feels incomplete, or sex remains painful. Once a clinician confirms that recovery is progressing, slower penetration and a generous water-based lubricant may reduce friction. Do not insert anything into the vagina if your maternity team has advised continued pelvic rest.

Seek urgent care for bleeding that is heavy or rapidly increasing, large clots, faintness, marked weakness, shortness of breath, severe abdominal or pelvic pain, fever, or foul-smelling discharge. These symptoms require assessment based on the bleeding and your postpartum state, regardless of whether intercourse happened just before them.

Contact your maternity team promptly if a wound opens, stitches appear disrupted, or bleeding resumes after it had settled. If you feel faint, collapse, or cannot safely travel, call local emergency services.

Ask whether the blood is coming from a healing tear, the vaginal wall, the cervix, or the uterus; whether there are signs of infection; and when penetrative sex is appropriate for your recovery. You can also ask which lubricant or moisturiser is suitable during breastfeeding and whether painful scar tissue needs treatment.
