Bleeding After Sex Following a Long Break

A small surface graze is more likely when penetration is rushed, lubrication is limited, or vaginal tissue is sensitive. The vagina does not become sealed or remain tighter from not having intercourse, so ongoing bleeding should be checked for another cause.

Dryness can be linked to limited arousal, breastfeeding, menopause-related hormonal change, or some medicines. Genitourinary syndrome of menopause often presents with vaginal dryness, burning, discomfort during sex, and sometimes spotting from fragile tissue.

The cervix can bleed on contact because of ectropion, inflammation, or a polyp. Chlamydia can present with bleeding after sex and may not cause obvious discharge or pain. Cervical dysplasia and cervical cancer can also present this way, although bleeding alone cannot show which explanation applies.

Allow the bleeding and soreness to settle before trying penetration again. Spend longer on arousal, use generous water-based lubricant, begin slowly, and choose a position where you control depth and pace. Stop rather than pushing through sharp, burning, or deep pain.

Note whether blood appears immediately after contact or continues later, and whether it coincides with a period. Book an appointment if the episode is unexplained, repeats, or occurs after menopause. Depending on your situation, assessment may include examination of the vagina and cervix, infection testing, pregnancy testing, or review of cervical screening.

Obtain urgent help for heavy bleeding, faintness, severe pelvic pain, or unusual bleeding with a missed period and abdominal pain. These features can point to significant blood loss or a pregnancy-related problem and should not be watched at home.

Arrange prompt clinical review for repeated contact bleeding, bleeding between periods, any bleeding after menopause, foul-smelling discharge, fever, pelvic pain, or possible exposure to a sexually transmitted infection. Even when friction seems plausible, recurrence changes the need for assessment.

Ask whether dryness, a visible graze, cervical change, or infection could account for the bleeding. Clarify whether you need swabs or updated screening, and ask about vaginal moisturisers, lubricants, or prescription treatment if hormonal dryness is making sex uncomfortable.
