What should you know about bleeding after sex 2nd trimester?

Pregnancy increases blood flow to the cervix, making it more likely to bleed after contact. That is one possible explanation, but a clinician needs to consider the amount, pain, pregnancy history, and whether the blood could be coming from the placenta or another source.

A sensitive cervix, a cervical polyp, vaginal irritation, or infection can produce spotting after penetration. Cervical inflammation may also cause unusual discharge, odour, pelvic discomfort, or urinary symptoms, although infection can be present without these signs.

Bleeding in the second trimester can also relate to the placenta, changes at the cervix, or pregnancy loss. Sex may simply make bleeding noticeable rather than cause the underlying problem. The appearance or volume of blood cannot safely exclude a pregnancy-related cause.

Stop intercourse, use a pad to monitor the flow, and call your maternity unit, obstetric clinician, or pregnancy assessment service for advice. Tell them when it started, the colour and amount, whether it continues, and whether you have pain, tightening, fluid leakage, fever, dizziness, or a change in the baby's usual movements if movements are already familiar.

Do not use tampons or insert anything into the vagina while awaiting guidance. Your care team may recommend an examination, ultrasound, or tests based on the circumstances. If you know that your blood group is Rh negative, mention this promptly because bleeding can affect whether additional treatment is considered.

Seek emergency maternity care for heavy or increasing bleeding, severe or persistent abdominal pain, regular tightening, fluid leaking from the vagina, fainting, marked dizziness, fever, or feeling seriously unwell. Also seek urgent assessment for a clear reduction in your baby's usual movements once you have established a movement pattern.

Ask where the bleeding seems to originate, whether the placenta and cervix need assessment, and whether infection testing is appropriate. Confirm whether you should avoid penetrative sex, for how long, and which symptoms should lead directly to the maternity assessment unit rather than a routine appointment.
