Bleeding after sex during pregnancy

The pregnant cervix has a rich blood supply and may spot after contact. That explanation cannot be confirmed from timing alone, and assessment depends on the amount of blood, stage of pregnancy, pain, and other symptoms.

Fresh blood noticed after penetration may come from the cervix or a small vaginal abrasion. Cervical inflammation, including inflammation caused by chlamydia, can also bleed on contact. Pregnancy-related bleeding may instead come from inside the uterus and happen coincidentally after sex.

Placental problems, pregnancy loss, or labour can present with vaginal bleeding, sometimes alongside pain, contractions, fluid leakage, or pressure. Bleeding after sex does not identify which explanation applies, and the symptom should be triaged by a maternity professional.

Stop sex and use a pad rather than an internal product so you can observe the bleeding. Contact your maternity unit, obstetrician, or other pregnancy-care clinician for advice, even if spotting settles. Tell them your stage of pregnancy, blood colour and amount, pain, contractions, fluid loss, and fetal movement if you usually feel it.

Follow the maternity team's guidance about whether to avoid further penetration and where to be examined. If you know that you have a rhesus-negative blood group, mention it when reporting bleeding because the clinical team may need to review your care.

Seek urgent maternity or emergency care for heavy bleeding, severe or one-sided abdominal pain, shoulder-tip pain, faintness, weakness, regular painful contractions, fluid leaking from the vagina, fever, or reduced fetal movement once movements are established.

Call emergency services if you collapse, struggle to breathe, feel confused, or cannot travel safely. Do not delay because the bleeding began after sex; warning symptoms determine urgency.

Ask whether you need an examination, ultrasound, blood-group review, or infection testing. Clarify whether sex and exercise are currently suitable, how to monitor the bleeding, and which changes require you to return immediately.
