Bleeding after postpartum sex

Childbirth, stitches, tears, and hormonal changes can leave the vagina tender. Sex may irritate the area before healing is complete. Bleeding can also be unrelated to delivery, so the pattern and accompanying symptoms matter.

Low oestrogen, especially while breastfeeding, may reduce natural lubrication and make vaginal tissue more vulnerable to friction. A healing tear, episiotomy, or tender scar can also spot after penetration. Postpartum discharge from the uterus can make the source of blood difficult to identify.

Other possibilities include infection, a cervical polyp, or inflammation of the cervix. Chlamydia can cause cervical inflammation and bleeding after sex, although bleeding alone does not show whether an infection is present. Cervical cancer can present with bleeding after sex, but only clinical assessment can establish the cause.

Pause penetration until bleeding and soreness have settled. When you feel ready again, go slowly, use a compatible lubricant, and stop if pain or fresh bleeding occurs. Avoid placing products inside the vagina when infection or an unhealed wound is possible.

Contact your maternity team, GP, or gynaecologist if the bleeding repeats, pain continues, or you are unsure that tears and stitches have healed. Describe the delivery, current discharge, contraception, breastfeeding, and any new sexual exposure so the clinician can choose an appropriate examination and tests.

Get urgent care for heavy or increasing bleeding, large clots, faintness, racing heartbeat, severe pelvic or abdominal pain, fever, or foul-smelling discharge. A wound that opens, becomes increasingly swollen, or drains pus also requires prompt attention.

Call emergency services for collapse, breathing difficulty, chest pain, or confusion. These warning signs require assessment regardless of whether sex appeared to trigger the bleeding.

You might ask whether the birth injury has healed, whether dryness is contributing, and which lubricant or moisturiser suits breastfeeding. Ask whether cervical or infection testing is indicated and what signs would mean you should return sooner.
