Bleeding during sex just after a period

Sex can bring remaining blood from the vagina into view, so timing close to a period can blur the distinction between menstrual and postcoital bleeding. New bright-red blood, bleeding that continues, or the same pattern in later cycles deserves clinical assessment rather than being assumed to belong to the period.

Dark red or brown spotting immediately after a period may be older blood leaving the vagina. Fresh bleeding can come from a small friction injury, vaginal dryness, an inflamed cervix, cervical ectropion, or a polyp. An infection such as chlamydia may present with bleeding after sex and can occur with discharge, pelvic pain, urinary discomfort, or no other symptoms.

Unusual contact bleeding is also a possible presentation of cervical dysplasia, cervical cancer, or vaginal cancer. That direction matters: these conditions may cause the symptom, but the symptom does not reveal which condition is present. A clinician locates the bleeding and uses your examination and tests to narrow the explanation.

Stop sex while actively bleeding or sore, and use a pad to observe whether the flow stops or continues. Write down the last day of your period, whether the blood looked brown or fresh red, the amount, pain location, discharge, odour, contraception, and whether pregnancy could be possible. This helps separate a late part of the period from bleeding triggered by contact.

Seek a routine appointment for any unexplained episode of bleeding during or after sex, and arrange review sooner if it recurs. Assessment may include a pregnancy test, infection testing, and examination of the vulva, vagina, and cervix. If a clinician finds a cervical change, they will explain whether closer examination or another procedure is needed.

Urgent assessment is needed for heavy bleeding, severe or one-sided pelvic pain, faintness, significant dizziness, shoulder-tip pain, fever, or rapidly worsening symptoms. If the recent period was lighter or different from usual and pregnancy is possible, bleeding with pelvic pain must be evaluated urgently. Vaginal bleeding after menopause should always receive prompt medical attention.

Ask whether the colour and timing fit residual menstrual blood, whether the cervix or vaginal wall bled during examination, and whether infection or pregnancy tests are advisable. Check whether your cervical screening is current and ask what follow-up is needed if bleeding returns. Mention medicines and hormonal contraception because they can alter bleeding patterns.
