Bleeding after painful sex: causes and next steps

Stop if penetration hurts. A single light spot after obvious friction may settle, but pain plus unexplained bleeding warrants an appointment, especially when symptoms recur or are accompanied by discharge, fever, urinary discomfort, or pelvic pain away from sex.

Pain at the vaginal entrance with a small amount of bright blood can follow dryness, a superficial tear, or irritation. Lower oestrogen around menopause may make vaginal tissue thinner, drier, and easier to injure. Deeper pain can arise when inflamed pelvic structures are moved during penetration, while a tender or inflamed cervix may bleed on contact.

Chlamydia can present with postcoital bleeding, pelvic discomfort, or unusual discharge. Cervical ectropion and polyps are other possible explanations. Cervical dysplasia, cervical cancer, and vaginal cancer can present with bleeding after sex, but neither the pain nor the blood can establish the cause without examination.

Avoid further penetration until soreness and bleeding have resolved. If you resume later, allow more time for arousal, use a compatible lubricant, and stop rather than pushing through pain. Keep a brief symptom record that distinguishes pain at the opening from pain deeper in the pelvis and notes the bleeding amount, cycle timing, discharge, contraception, and pregnancy possibility.

Book a medical or sexual health assessment. The clinician may examine the vulva for a tear, inspect the vagina and cervix, and offer swabs, urine testing, or a pregnancy test according to your symptoms. Tell them if cervical screening is overdue, while recognising that screening is not a diagnostic test for active bleeding.

Get urgent help if bleeding is heavy, pain is severe or worsening, you faint or feel very dizzy, you develop shoulder-tip pain, fever, vomiting, or feel acutely unwell. Possible pregnancy together with bleeding and pelvic or abdominal pain needs urgent evaluation. Bleeding after menopause also requires prompt medical review, regardless of whether sex seemed to trigger it.

Describe exactly where the pain occurs and ask whether the examination shows dryness, injury, inflammation, or a cervical change. Ask which infection tests are relevant, whether pregnancy should be excluded, and whether an ultrasound or specialist cervical examination is appropriate. If dryness is suspected, discuss moisturisers, lubricants, and whether local hormonal treatment suits your health history.
