Bleeding during or after sex: causes and next steps

A single small spot associated with dryness may settle, but bleeding that repeats, has no clear friction trigger, or accompanies pain or unusual discharge should be checked by a clinician.

Blood noticed during penetration can arise from the vulva, vaginal wall, or cervix. Friction may injure dry tissue, while inflammation or infection can make the cervix bleed when touched. Menstrual blood that was already present may also become noticeable during sex.

Hormone-related thinning in genitourinary syndrome of menopause can lead to dryness and contact bleeding. Chlamydia can cause cervical inflammation and bleeding after sex. Cervical dysplasia, cervical cancer, and vaginal cancer can present with contact bleeding, although the bleeding pattern cannot distinguish among causes.

Stop sexual activity if bleeding or pain begins. Note the amount and colour of blood, whether it continues, your menstrual timing, any pregnancy possibility, and associated discharge, odour, sores, urinary symptoms, or pelvic pain.

If dryness seemed involved, let irritated tissue recover before trying penetration again and use ample compatible lubricant. Do not douche or insert products to clean away blood. Arrange an examination for recurrent bleeding; the clinician may inspect the vagina and cervix and recommend infection or cervical testing based on your history.

Get urgent medical help if bleeding is heavy or does not ease, or if it occurs with severe abdominal or pelvic pain, fainting, substantial dizziness, weakness, fever, or possible pregnancy. Injury from forced or traumatic penetration also warrants prompt, confidential care.

Arrange a timely clinic visit when bleeding returns, occurs between periods as well as after sex, begins after menopause, or accompanies painful sex or abnormal discharge. Cervical screening does not replace assessment of a new symptom.
