Bleeding after dry sex: what it may mean and what to do

Pause penetration until soreness and bleeding have settled. If bleeding is more than light spotting, keeps returning, or occurs with significant pain, arrange medical care rather than assuming dryness is the only cause.

Without enough lubrication, penetration can rub the vaginal opening, vaginal walls, or vulval skin. The irritated surface may sting, feel raw, and leave a small amount of pink or red blood on tissue or bedding.

Dryness may occur with limited arousal, hormonal changes around menopause, breastfeeding, or some medicines. Genitourinary syndrome of menopause can involve thinner, drier tissue that bleeds more readily with contact. Chlamydia and changes affecting the cervix or vagina can also present with bleeding after sex, so repeated episodes deserve assessment.

Avoid penetration and anything that rubs the sore area while it heals. Wash gently with water, avoid scented washes or internal cleansing, and use a pad rather than inserting a tampon if you need to monitor spotting.

When you resume sex, allow more time for arousal, communicate about discomfort, and use a generous amount of compatible water-based or silicone-based lubricant. Stop if pain or fresh bleeding begins. A clinician can check for a tear, infection, cervical inflammation, or hormone-related dryness when symptoms recur.

Seek urgent care for heavy or continuing bleeding, severe pelvic or abdominal pain, fainting, marked dizziness, weakness, or any possibility of pregnancy with pain or bleeding. These features should not be managed as simple friction at home.

Book a non-urgent appointment if spotting happens again, occurs without sex, follows menopause, or comes with unusual discharge, pelvic discomfort, pain during sex, sores, or urinary symptoms. Examination and appropriate infection or cervical testing can identify the source.

Ask where the bleeding appears to come from, whether the tissue shows friction injury or hormonal dryness, and whether infection testing or a cervical check is appropriate. Mention medicines, contraception, menstrual timing, pregnancy possibility, menopause, breastfeeding, discharge, and pain.
