Bleeding After a Year Without a Period

When the year-long gap represents the end of natural periods, new bleeding is classed as postmenopausal bleeding. It may come from delicate vaginal tissue, the cervix, or the womb. Causes include tissue changes related to lower oestrogen, polyps, infection, altered womb lining, and medicines or hormone therapy. Womb or cervical cancer can also present with bleeding after menopause, so assessment should not be delayed.

For someone whose periods stopped because of contraception, pregnancy, illness, low body weight, intensive exercise, a hormone condition, or ovarian insufficiency, the interpretation may differ. The clinician first confirms the source and the reason periods had stopped.

Contact a gynaecology or primary-care service promptly. Before the visit, write down the date, colour, duration, and approximate amount of bleeding, plus any pelvic pain, unusual discharge, urinary symptoms, or bleeding after sex. Take the names of prescribed and non-prescribed medicines, especially hormones and medicines that affect clotting.

A clinician may examine the vagina and cervix, use transvaginal ultrasound to view the womb lining, and take a small tissue sample if indicated. Depending on your circumstances, pregnancy testing, cervical testing, swabs, or blood tests may also be considered.

Go to emergency care for bleeding that is rapidly soaking pads, severe or worsening abdominal or pelvic pain, fainting, pronounced weakness, dizziness, difficulty breathing, chest pain, confusion, or pale and clammy skin. If pregnancy could be possible, bleeding with one-sided pain, shoulder pain, or collapse also requires emergency attention.

If you feel stable and the bleeding is slight, arrange a prompt clinic visit. Do not treat the absence of pain or the brown colour of old blood as reassurance that no assessment is needed.

You might ask whether the bleeding is from the vagina, cervix, urinary tract, or womb; what could have caused the year without periods; which tests will clarify the cause; and when results will be available. Confirm whom to contact if another episode occurs while you are waiting.
