Symptoms when bladder cancer affects the kidney or ureter

These problems do not prove that cancer has entered kidney tissue. A bladder tumour can obstruct a ureter, the tube carrying urine from a kidney, and cause urine to back up. Imaging, urine and blood tests are needed to distinguish obstruction, infection, stones, treatment effects and cancer spread.

The bladder sits downstream from the kidneys. Cancer near a ureter opening can narrow urine flow and enlarge the collecting system above it, a problem called hydronephrosis. Cancer may also extend into nearby structures or appear elsewhere in the urinary tract, but the location must be confirmed rather than assumed from symptoms.

Possible signs include a dull or sharp ache in the side or back below the ribs, blood in urine, burning, difficulty passing urine, fever, chills, nausea, swelling or passing much less urine. Impaired kidney function can initially cause few noticeable changes, which is why blood tests and scans matter.

Contact the treating team about new flank pain, visible blood in urine, fever or a clear change in urine output. Note when symptoms began, whether pain comes in waves, and whether vomiting, chills or urinary burning accompany it. Bring details of current cancer therapy and all medicines, including non-prescription pain relief.

Assessment may include kidney-function blood tests, a urine sample and imaging of the urinary tract. If urine flow is blocked, clinicians may drain the kidney with a ureteric stent or a tube placed through the back. Further treatment is selected according to the obstruction's cause, the cancer's extent and the person's health.

Get urgent medical help for fever or shaking chills with side pain, very little or no urine, severe or rapidly worsening pain, persistent vomiting, fainting, confusion or marked weakness. An infected blocked kidney can become serious quickly, and inability to pass urine also needs prompt assessment.

Call the oncology service without delay if symptoms begin during cancer treatment, especially when the team has given specific fever instructions. Visible blood or clots with difficulty urinating should not be saved for the next scheduled visit.

Consider asking: Is the kidney itself involved, or is urine drainage blocked? What did imaging show about the ureter? Are my kidney blood results changing? Do I need drainage before cancer treatment continues? Which symptoms mean I should go directly to emergency care? Also ask whether any current medicine should be adjusted while kidney function is being assessed.
