Blackheads on the Nose: Treatment and Care

Short answer
Blackheads on the nose often improve with gentle cleansing, salicylic acid, or a suitable topical retinoid used consistently.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Medical name
- Open comedones
- Dark colour
- Oxidised material at the open pore, not dirt
- Common first options
- Gentle cleansing, salicylic acid, and a suitable topical retinoid
The short answer
Avoid forceful squeezing or harsh scrubs, which can inflame the skin without preventing the pore from filling again.
Why blackheads form on the nose
A blackhead is an open comedone: oil and dead skin cells collect inside a hair follicle while its opening remains exposed. The dark surface comes from oxidation on contact with air, not trapped dirt.
Small, evenly spaced dots across the nose may instead be sebaceous filaments, normal structures that guide oil to the skin surface. They can look less noticeable after cleansing or exfoliation but commonly refill. A dermatologist can distinguish them from comedonal acne.
A practical treatment approach
Wash with a mild, non-comedogenic cleanser and use a leave-on salicylic acid product if your skin tolerates it. Salicylic acid helps loosen material within oily follicles. Introduce one active product gradually, then add an oil-free moisturiser and daytime sunscreen to limit dryness and irritation.
A topical retinoid can reduce new comedones by normalising how cells shed inside follicles. Apply it as directed across the blackhead-prone area rather than dabbing only visible spots. Retinoids may irritate skin and require professional guidance during pregnancy or while trying to conceive.
Professional extraction can clear selected blackheads temporarily, but it does not stop new plugs forming. Sterile technique matters because home extraction tools and fingernails can cause inflammation, pigment changes, or scarring. Oral isotretinoin is generally reserved for acne that warrants specialist systemic treatment, rather than a few nose blackheads alone.
When professional care is useful
Arrange a dermatology assessment if the bumps become painful, red, swollen, or pus-filled; if they leave scars or persistent dark marks; or if consistent over-the-counter care causes irritation without improvement. Seek prompt care for rapidly spreading redness, marked swelling, or fever, as these features are not typical of simple blackheads.
Questions to ask your dermatologist
Ask whether the dots are blackheads or sebaceous filaments, which active ingredient suits your skin sensitivity, how to combine treatment with your current products, and whether extraction would add meaningful benefit. Mention eczema, rosacea, pregnancy, breastfeeding, or medicines that already make your skin dry.

Questions people ask
Can I squeeze blackheads on my nose?
Squeezing can injure the follicle and trigger inflammation, pigment changes, or scarring. If extraction is appropriate, a trained professional can use sterile tools and controlled pressure.
Are nose pore strips a long-term treatment?
Pore strips may lift surface material and briefly change how pores look, but they do not correct the buildup process inside follicles. They can also irritate sensitive skin.
Why do the dark dots return after cleansing?
Follicles continue producing oil and shedding cells, so plugs can reform. If the dots are sebaceous filaments, refilling is part of their normal function rather than treatment failure.
