The short answer
They form when oil and dead cells remain at a follicle opening. Air exposure darkens the plug; the colour is not evidence that the skin is dirty.
Why the chest is affected
The chest contains many oil-producing follicles and is often covered by clothing. Sweat, close-fitting sportswear, straps and oily body products can create an occlusive or irritating environment in which follicle plugs persist.
Open comedones occur in acne and can dominate comedonal acne. Uniform itchy bumps, pus-filled spots or a sudden rash may indicate folliculitis or another condition instead, particularly when there are few true dark plugs.
Body care that may help
Rinse after heavy sweating, change damp tops and wash the chest with a gentle cleanser without abrasive scrubbing. Select non-comedogenic sunscreen and body products, and wash workout clothing between uses.
A topical retinoid may help keep follicles clear, although chest skin can still become dry or irritated. Follow the recommended amount and frequency rather than applying extra. Obtain clinical advice before use during pregnancy or pregnancy planning, and keep treatment away from raw or sunburned skin.
When to arrange care
See a clinician when the bumps are painful, very itchy, pus-filled, rapidly spreading or leaving raised scars or persistent dark marks. Fever, worsening warmth, expanding redness or feeling generally unwell calls for prompt assessment because infection may need treatment.
Routine review is also worthwhile if over-the-counter care irritates the chest, the spots keep returning or you cannot tell blackheads from a rash. Extensive inflammatory acne can require prescription management; isotretinoin is considered only after medical evaluation and monitoring.
