Recognising a blackhead

An open comedone looks like a dark plug at the mouth of a follicle. The colour is caused by exposed material in the pore reacting with air, not by trapped dirt. Simple comedones are generally less inflamed than red pimples or pus-filled follicles.

A curved hair may be visible in an ingrown-hair bump, especially after close hair removal. Firm lumps under the skin, recurring tender swellings, open drainage points or rope-like scars represent a different pattern and deserve examination rather than home extraction.

Reduce friction and blockage

Clean the external groin gently and dry it without rubbing. Loose underwear and clothing can reduce repeated friction. If bumps follow hair removal, let irritated skin settle and reconsider close shaving; use clean equipment and avoid passing a blade repeatedly over the same patch.

Keep fragranced products, harsh scrubs and acne treatments away from the genital surface. Picking with fingernails or tools can push inflammation deeper. If treatment is needed, a clinician can identify the affected skin precisely and decide whether a comedone medicine, an approach for ingrown hairs or another plan fits the diagnosis.

When an examination matters

Book an appointment when the bumps keep returning, become painful, release pus or blood, develop an odour, or heal with thickened scars. A new dark spot that does not look like a follicle, changes in shape or colour, or bleeds without being picked should also be examined.

Get prompt help if redness is spreading, the area becomes very hot or swollen, pain escalates, or fever and general illness develop. These are warning features because they can signal a spreading skin infection, regardless of what first caused the bump.

Prepare for a clinical review

Tell the clinician when the spots began, whether they coincide with shaving, exercise or particular clothing, and whether similar lumps occur elsewhere. Ask how to distinguish an open comedone from an ingrown hair and which skin boundaries must be avoided when applying treatment.

If episodes recur, photographs taken before drainage or healing may help show the pattern. Ask whether your current cleanser, moisturiser and hair-removal routine could be aggravating the follicles.
