Blackheads and whiteheads on the face

Together they are called comedones and are a common presentation of comedonal acne. Their appearance does not indicate dirty skin, and aggressive cleaning can make the surrounding skin sore without clearing the blockage.

Oil and shed skin cells collect within a follicle. When the opening stays exposed, the material at the surface darkens and forms a blackhead. When a thin layer of skin covers the opening, the bump looks pale or flesh-coloured and is called a whitehead.

Comedones often cluster on the forehead, nose, chin, or cheeks, where oil production and contact with cosmetics or hair products may contribute. Red, tender spots indicate added inflammation. Tiny uniform bumps, irritation around the mouth, or an itchy rash may represent another condition and deserve a closer look.

Cleanse without brushes or abrasive grains, remove makeup before sleep, and select non-comedogenic moisturiser and sunscreen. Keep heavy hair oils and styling products away from acne-prone facial skin. Do not pierce whiteheads or squeeze blackheads, as trauma can lead to inflammation and lasting marks.

Topical retinoids can help normalise cell turnover inside follicles when medically appropriate. Follow instructions carefully and discuss pregnancy or pregnancy plans before use. A trained professional may extract selected comedones with sterile technique, but maintenance treatment is usually needed because extraction alone does not address formation of new plugs.

Consult a dermatologist if comedones persist, become inflamed, leave dark marks or scars, or affect how you feel about your appearance. Arrange prompt assessment for severe swelling, rapidly spreading redness, intense pain, fever, or involvement close to the eye. These features go beyond uncomplicated blackheads and whiteheads.

Bring a list or photographs of cosmetics, sunscreen, hair products, and treatments already tried. The clinician can check for pore-clogging exposures and design a routine suited to skin sensitivity and the types of acne present.

Ask whether all the bumps are comedones, which product should be introduced first, and how to respond if peeling occurs. If extraction or stronger treatment is proposed, clarify its purpose, aftercare, pregnancy precautions, and how recurrence will be limited.
