Black or very dark uneven skin tone: what to consider

The pattern matters: marks left after acne or eczema differ from symmetrical facial patches, scattered sun spots, or one changing lesion. Treatment should address the cause and avoid irritation, which can deepen pigment, particularly in skin that readily develops dark marks.

Post-inflammatory hyperpigmentation appears where skin was previously inflamed or injured. Melasma often presents as broader, fairly symmetrical facial pigmentation, while sun-related change can look mottled or form defined spots on exposed areas. Repeated rubbing may darken folds or pressure points. A new product can cause irritation first and pigment afterward.

Velvety thickening in body folds, widespread colour change, or pigmentation involving the mouth or nails can have different explanations and merits medical assessment. A single very dark spot that evolves should be evaluated as a lesion, not treated as general uneven tone.

Start with a gentle cleanser, moisturiser, and broad-spectrum sun protection on exposed skin. Limit rubbing, harsh scrubs, bleaching mixtures, and frequent product changes. Control acne, eczema, or another active rash because repeated inflammation can create fresh marks even while older pigment fades.

A dermatologist can match topical treatment or a procedure to the diagnosis, skin tone, depth of pigment, and tendency to mark. Chemical peels, skin-brightening approaches, and light-based procedures require careful selection; excessive strength or heat can worsen discolouration. Patch testing and a staged plan help reveal irritation early.

Arrange an appointment for a new patch that grows, changes shape or colour, develops irregular borders, bleeds, crusts, ulcerates, or does not heal. Rapidly spreading discolouration, pain, marked itch, swelling, blistering, or symptoms beginning after a medicine also deserve assessment.

Seek urgent care when pigment change accompanies facial swelling, breathing difficulty, extensive blistering, skin peeling, or sores of the eyes or mouth. Stop a newly applied cosmetic if it causes burning or a severe rash, and bring its ingredient list to the consultation.
