Black Stool in an Elderly Woman

A black, sticky, tar-like bowel movement may contain blood that has been digested in the stomach or upper bowel. Gastritis and a peptic ulcer are possible causes; Helicobacter Pylori Infection can contribute to irritation and ulcer disease. Iron tablets, bismuth medicines, and dark foods can also make stool look black, but they do not safely explain every change. Older adults may have little pain despite significant bleeding, particularly when taking several medicines or medicines that irritate the stomach.

Arrange same-day medical advice and describe the colour, stickiness, smell, and number of bowel movements. Bring a list of medicines, including aspirin, anti-inflammatory pain medicines, blood thinners, iron, and supplements. Do not change a prescribed blood thinner or pain medicine without medical direction. If possible, keep a photo for the clinician. Eat and drink only as tolerated while arranging care, and do not wait for the stool to return to its usual colour before seeking advice.

Call emergency services for fainting, new confusion, severe weakness, breathlessness, chest discomfort, cold clammy skin, vomiting blood, severe abdominal pain, or repeated black stools. A rapid heartbeat, very little urine, or inability to stand safely also needs immediate help. Have another person stay with her and bring her medication containers or list.

Ask whether gastritis, Helicobacter Pylori Infection, or a peptic ulcer could explain the finding; whether blood tests or other assessment are needed; and which medicines should be continued. Tell the clinician about previous ulcers, stomach symptoms, weight change, appetite, vomiting, dizziness, and the timing of the stool change.
