Black spots under the eyes: causes and next steps

The appearance matters: tiny raised dots can differ from flat brown-grey patches, a bruise-like area, or a single changing mark. Because eyelid skin is thin and sensitive, avoid treating the area as ordinary facial acne before the cause is clear.

Small dark plugs at follicle openings can be open comedones. Flat discoloration may follow rubbing, eczema, a cosmetic reaction, a pimple, or other inflammation; pigment can remain after the irritation settles. Shadows from tear-trough shape or visible vessels may look dark without being discrete spots.

A sharply defined mole-like mark deserves attention if it is new or evolving. Note whether the colour is uniform, whether the surface is smooth, and whether there is scaling, tenderness, itching, swelling, discharge, or an eye symptom.

Clean gently with lukewarm water and a fragrance-free product, then use a simple moisturiser suitable for the eye area. Stop any new cosmetic that coincided with burning, itching, or darkening. Do not squeeze dots or apply scrubs, bleaching mixtures, acne acids, or retinoids close to the eyelid margin unless a clinician has advised the exact product and placement.

Take a clear photograph in similar lighting to track a focal mark. A dermatologist can examine pigment and follicles closely and decide whether treatment should target comedones, irritation, or excess pigment.

Arrange a medical review for a spot that grows, changes shape or colour, repeatedly crusts or bleeds, or does not settle after an obvious irritation has stopped. Seek prompt care for painful eyelid swelling, spreading redness, pus, fever, eye pain, light sensitivity, double vision, or reduced vision. Sudden darkening after an injury also warrants assessment, especially with headache, vomiting, or visual disturbance.

Ask whether the marks are within pores or are pigment in the skin, which products should be kept away from the eyelid, and whether an eye specialist is needed. If treatment is offered, clarify the likely irritation risk and how to protect this delicate area from sun exposure.
