Black spots on the penis: possible explanations and care

Colour, surface, location, symptoms, and change over time are more informative than darkness alone. A clinician may need to inspect the area because photographs and descriptions cannot reliably distinguish every cause.

Some marks are stable natural pigmentation or harmless pigment spots. Darkening may also remain after friction, shaving, follicle inflammation, a rash, or another minor injury has healed. A dark dot within a tender or pus-filled bump is more suggestive of an inflamed follicle than a simple flat pigment mark.

A rough, raised, ulcerated, clustered, or newly multiplying lesion needs direct assessment. Sexual contact history, itch, pain, discharge, sores, and urinary symptoms help a clinician decide whether infection testing is appropriate.

Wash gently with water or a mild unscented cleanser, dry by patting, and reduce rubbing from tight clothing. Pause shaving if follicles are irritated. Do not squeeze, scrape, bleach, freeze, or apply facial acne acids to genital skin.

Note when the mark appeared and whether it changes; a private photograph can help track evolution for your own clinical visit. If infection is possible, avoid sexual contact until assessed, or use barrier protection and discuss appropriate testing with a qualified clinician.

Seek prompt care for severe pain, rapid swelling, spreading redness, fever, difficulty passing urine, pus, an open sore, or tissue that appears damaged. A lesion that bleeds without clear injury or changes in shape, border, colour, size, or surface also needs medical examination.

Arrange a routine appointment for any unexplained new mark, several new lesions, persistent itch, recurrent inflammation, or a spot that does not settle after an obvious irritation has stopped.

Tell the clinician about onset, shaving or friction, medicines applied to the area, sexual exposure, and associated sores or discharge. Ask whether the lesion is pigment, follicle-related, infectious, or another growth; whether testing is needed; and which genital-safe treatment is appropriate.
