Black spots on feet and ankles: causes and warning signs

A flat mark left after a bite, rash, pimple, scratch, or shoe rubbing is often post-inflammatory pigmentation. A clinician should assess a new dark lesion that is changing, a black area within a wound, or spots accompanied by pain, swelling, warmth, numbness, or fever.

Texture and pattern offer useful clues. Smooth brown or grey marks may follow inflammation or repeated rubbing. Pinpoint red-purple marks that darken can reflect small amounts of bleeding beneath the skin. Darkening around swollen ankles may occur when blood does not return efficiently through the leg veins.

Tender bumps around hairs, especially after shaving or sweating, can occur with folliculitis and may leave dark marks as they settle. A single mole-like spot deserves attention if it is new, uneven in shape or colour, enlarging, bleeding, crusting, or unlike the person's other marks. Appearance alone cannot confirm the cause.

Check both feet in good light, including the soles, between the toes, and around the nails. Note whether each mark is flat or raised, whether pressing changes its colour, and whether there is itching, scaling, a lump, broken skin, discharge, or ankle swelling. A dated photograph beside a ruler can help show change over time.

Avoid picking, scrubbing, bleaching products, or applying steroid creams without a diagnosis. Reduce rubbing from tight footwear, keep broken skin clean and covered, and arrange a routine medical or dermatology appointment if the cause is unclear, the marks persist, or they continue to change.

Seek urgent care for a black or purple area that appears suddenly with severe pain, rapidly increasing swelling, spreading redness, fever, pus, or red streaks. A foot that becomes cold, pale, blue, numb, or weak also needs urgent assessment because blood flow or nerve function may be affected.

Prompt assessment is also sensible for a dark area inside an ulcer, a wound that is worsening, or any foot injury in someone with diabetes, reduced sensation, or known circulation disease. Book a dermatology review for a changing or bleeding mole-like lesion even when it does not hurt.

Ask whether the pattern looks more like pigment after inflammation, bleeding beneath the skin, a circulation-related change, infection, or a mole. Also ask whether the skin needs examination with a dermatoscope, whether circulation or sensation should be checked, and which products are safe while the diagnosis is being clarified.
