Black seed oil and tamoxifen: what to know

Black seed oil, also called Nigella sativa oil, contains active plant compounds and is not automatically harmless because it is sold as a natural product. Information about its use alongside tamoxifen is limited, so a clinician needs to weigh the reason you want it, the product ingredients, your other medicines, and your health history.

Tamoxifen is processed by enzymes in the liver into compounds that contribute to its treatment effect. Black seed products may influence medicine-processing pathways, but the effect in a particular person and product is uncertain. This uncertainty is the reason for checking the combination, not proof that an interaction will occur.

Capsules, bottled oils, powders, and mixed herbal formulas can contain different ingredients and strengths. A pharmacist can assess the exact label and also check for overlap with prescription medicines, non-prescription medicines, vitamins, and other supplements. Do not stop tamoxifen or change its schedule to make room for a supplement unless the prescriber directs you to do so.

Show the care team the container or a clear photo of the full ingredient panel, suggested serving, manufacturer, and any quality certification. Explain why you want to take it and whether you have already used it. Include all medicines and supplements, particularly those for mood, pain, blood clotting, blood pressure, or blood sugar, because the combined medication list shapes the advice.

If you already took black seed oil, note the product, amount, timing, and any new symptoms. Contact the clinician or pharmacist who manages your tamoxifen for individual guidance. Keep taking tamoxifen as prescribed unless that clinician tells you to pause or change it.

Get emergency help for trouble breathing, swelling of the face or throat, fainting, sudden chest pain, coughing up blood, one-sided leg swelling, sudden weakness, difficulty speaking, or a sudden severe headache. These symptoms can signal a serious allergic reaction, a blood clot, or another urgent problem and should not wait for a routine appointment.

Contact your care team promptly for a new rash, persistent vomiting or diarrhoea, unusual bruising or bleeding, yellowing of the skin or eyes, dark urine, or symptoms that began after adding the oil. Bring the supplement packaging so the ingredients can be reviewed.

Ask whether any ingredient in your specific product could alter tamoxifen processing or add to side effects from your other medicines. You can also ask what symptom or goal you hope the supplement will address, whether a safer option exists, and how your team wants supplements recorded during hormone therapy.
