Black scaly spots on skin: causes and next steps

Colour and scale alone cannot identify the cause. The spot's history, surface, border, symptoms and changes help a clinician decide whether examination or testing is needed.

A rough dark patch may be built-up keratin, a healing crust, pigment left after inflammation, or blood within a scratched lesion. Acne, folliculitis and razor bumps can leave dark marks after the active bump settles, especially in richly pigmented skin.

A spot that feels waxy or stuck on may be a benign growth, while an irregular lesion that keeps evolving requires a closer look. Do not peel the scale or use its colour as a diagnosis; removing the surface can cause bleeding and obscure useful features.

Wash gently, pat dry and apply a plain fragrance-free moisturiser around dry skin. Avoid picking, scraping, acids, bleaching products and home freezing. Protect exposed areas from sunlight because irritation and ultraviolet exposure can deepen uneven pigmentation.

Note when the spot appeared and whether its size, outline, colour, thickness or sensation changes. A clear photograph beside a ruler can help document evolution, but it does not replace an in-person skin examination.

Arrange a dermatology assessment for a new dark scaly spot that persists, enlarges, develops several colours, has an uneven border, repeatedly crusts or bleeds, or looks different from your other marks. Prompt care is also appropriate for increasing pain, warmth, swelling, pus or spreading redness.

Seek urgent help if dark skin follows a significant burn, rapidly spreads with severe pain, or accompanies fever or feeling very unwell. These features need assessment for deeper injury or infection rather than cosmetic treatment.

Ask what features support the likely diagnosis, whether dermoscopy or a small skin sample would clarify it, and which products should be stopped. If removal is discussed, ask whether the tissue should be examined and how to reduce the chance of a lasting dark or light mark.
