Black Dots on Trichoscopy: What They Can Show

They are a visual clue rather than a diagnosis. A clinician interprets their pattern alongside symptoms, hair-pull findings, scaling, redness, scarring and the appearance of nearby follicles.

A black dot forms when a pigmented hair shaft breaks at or just below the opening of a follicle. Under magnification, the remaining fragment looks like a dark point. Multiple conditions can produce this sign, including active patchy hair loss, scalp fungal infection and repeated pulling or mechanical breakage.

Distribution helps narrow the possibilities. Smooth patches with short tapered hairs suggest a different process from an itchy, scaly area containing hairs of uneven length. Black dots can be harder to distinguish from crust, fibre or product residue without a clear magnified view.

Arrange a dermatology assessment if the dots accompany a new bald patch, continuing shedding, itch or scale. Avoid scratching, tight hairstyles and harsh chemical processing while the cause is being assessed. Do not share combs, hats or pillowcases when there is scaling or concern about a contagious scalp infection.

The clinician may examine other scalp areas, check whether hairs break easily and inspect skin or nails for related clues. They may collect hairs or scalp scale when infection is possible. Treatment should target the cause; the magnified sign itself does not need to be removed.

Seek prompt medical care for a painful or swollen scalp, pus, fever, rapidly enlarging bald areas, or tender lumps around the neck. Early assessment also matters when the skin looks shiny and follicle openings seem to be disappearing, because that can accompany scarring hair loss.

A child with black dots, broken hairs and scalp scale should be examined rather than treated only with cosmetic products. Some scalp infections require prescription medicine taken by mouth.

Ask which accompanying trichoscopy features support the suspected cause, whether testing for fungus is appropriate, and whether any follicles show signs of scarring. You can also ask how to care for the scalp while awaiting results and which changes would require an earlier follow-up.
