Biventricular Heart Failure Symptoms: What to Know

How the structures involved in Heart Failure differ from normal

Short answer

Biventricular heart failure can cause breathlessness, tiredness, cough, swelling and reduced ability to exercise because both sides of the heart are struggling to move blood effectively.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Main pattern
Breathing symptoms and fluid-related swelling may occur together.
Common changes to track
Breathlessness, ankle swelling, cough, fatigue and exercise tolerance.
Urgent warning
Severe breathlessness, fainting, chest pain or blue lips needs emergency help.

What these symptoms can mean

Biventricular heart failure means the heart's left and right ventricles are both affected. The left side normally sends oxygen-rich blood around the body; the right side sends blood towards the lungs. When pumping becomes less effective, fluid may build up in the lungs or in tissues such as the legs and ankles.

People may become short of breath during activity, when lying flat, or at night. They may also notice persistent cough, fatigue, dizziness, a fast or irregular heartbeat, reduced appetite, or swelling that leaves shoes and socks feeling tight. A symptom such as fatigue can have many causes, so it does not identify heart failure on its own. Heart failure is the condition that can present with these symptoms, not the other way around.

What to do next

Arrange a medical assessment if symptoms are new, worsening, or interfering with ordinary activities. A clinician may ask about breathing, sleep, swelling, weight changes, medicines and other heart conditions, then examine you and decide whether tests are needed.

Take prescribed medicines as directed and do not stop them without medical advice. Keep a simple record of changes in breathlessness, ankle swelling, energy, sleep and exercise tolerance. Tell your care team promptly about a noticeable change, especially after an infection or a change in treatment. A chronic disease review can help coordinate ongoing monitoring.

When to seek urgent care

Seek emergency help for severe or rapidly worsening breathlessness, breathing difficulty at rest, fainting, new confusion, blue or grey lips, or chest pain that is severe, persistent or accompanied by sweating or nausea. Call local emergency services rather than driving yourself if you are struggling to breathe or feel faint.

Urgent assessment is also appropriate for a sudden increase in swelling, a new irregular or racing heartbeat with weakness, or symptoms that prevent you from lying down or speaking comfortably. These changes need prompt assessment because fluid overload or a heart rhythm problem may require immediate treatment.

Questions for your doctor

Ask what may be causing your symptoms, how you should monitor changes at home, which symptoms require same-day help, and whether your medicines or daily routine need adjustment. You can also ask how other conditions, such as high blood pressure or an irregular rhythm, may affect your heart failure care.

Questions people ask

Can biventricular heart failure cause both breathing problems and swollen ankles?

Yes. Reduced pumping on the left can contribute to fluid in the lungs, while right-sided pressure can contribute to fluid collecting in the legs and ankles. Both patterns may be present when both ventricles are affected.

Do these symptoms always mean biventricular heart failure?

No. Breathlessness, cough, tiredness, palpitations and swelling have several possible causes. A clinician needs to assess the pattern, examine you and arrange appropriate tests.

What should I monitor between appointments?

Note changes in breathing, sleep position, swelling, energy, heartbeat sensations and ability to carry out usual activities, and report a clear deterioration to your care team.
