Choosing birth control for heavy periods

Hormonal birth control can reduce heavy periods, but the right method depends on your health and contraceptive needs. A hormonal coil often makes bleeding lighter over time, while combined pills and some progestogen methods may also help. Bleeding can be irregular at first, and not every contraceptive improves it.

Heavy menstrual bleeding describes blood loss that disrupts daily life, such as frequent product changes, leaking, passing large clots, or avoiding activities. Hormonal contraception may thin the womb lining or suppress ovulation, which can reduce bleeding. It can also mask the pattern without addressing a separate cause.

Ongoing blood loss can contribute to iron deficiency anaemia, which may cause tiredness, breathlessness, paleness, headaches, or a racing heartbeat.

Keep a record of bleeding days, flooding, clots, pain, bleeding between periods, and how symptoms affect sleep or work. Note whether you need pregnancy prevention, how you feel about an inserted device or daily tablet, and whether predictable bleeding matters to you. A clinician may check for pregnancy, anaemia, medication effects, or a womb-related cause before recommending treatment.

Mention migraine with aura, high blood pressure, clotting history, smoking, breast cancer history, liver disease, and medicines or supplements. These details can rule out oestrogen-containing options or influence which progestogen method is appropriate. Ask what bleeding changes to expect after starting and when the method should be reviewed.

Get urgent help if bleeding is very heavy and you feel faint, confused, severely weak, short of breath at rest, or have chest pain. Seek prompt assessment for heavy bleeding with severe pelvic pain, fever, a possible pregnancy, or bleeding after menopause. Arrange a routine appointment when heavy periods persist, begin unexpectedly, occur between periods, or cause symptoms of iron deficiency.

You could ask whether the bleeding pattern needs blood tests, an examination, or imaging; which options suit your medical history; how each choice may affect cramps and cycle control; and what the plan is if bleeding remains disruptive.
